All applicants who are offered employment at Dream Diner will be required to present documentation (prior to or on the first day of employment), verifying identity and authorization to work in the United States in accordance with the Immigration Reform and Control Act of 1986. You will not be employed or continued in Dream Diner’s employment if you are unable or unwilling to provide appropriate verification as Dream Diner may be required to request.

It is unlawful in Massachusetts to require or administer a lie detector test as a condition of employment or continued employment. An employer who violates this law shall be subject to criminal penalties and civil liability. Dream Diner complies with all applicable federal and state laws limiting the use of lie detector testing.

It is the policy of Dream Diner to maintain a workplace free of illegal drugs and misuse of alcohol and not knowingly hire anyone who uses illegal drugs. Applicants applying for a position or positions requiring the use or operating of heavy machinery or vehicles will undergo urinalysis drug testing. Any job offer from Dream Diner will be conditioned upon the applicant’s receiving a negative urinalysis drug test result.

1. I certify that the information in this application and in any other documents which I have provided to Dream Diner or one of its agents in the application process is complete and correct to the best of my knowledge, and I understand that falsification or omission of any information submitted during the application process is grounds for not being considered for employment, retraction of an extended job offer, or dismissal, if hired, in accordance with the policies of Dream Diner.

2. If required, I agree to submit to a physical examination and/or drug test, which will be conducted in a manner consistent with the requirements of applicable federal and state laws, and I understand that any employment offer made to me by Dream Diner is conditioned upon the results of such physical examination and/or drug test being acceptable to dream diner. I also understand that, if I am hired the Company reserves the right to test employees for drug and alcohol use in accordance with Company policies regarding such testing.

3. I understand that this application is not a contract for employment with dream diner.
